- If
your horse needs colic surgery, Farnam will reimburse the surgeon
and pay for up to three days of aftercare, up to $5,000, when
program conditions are met
- To
be eligible to enroll, treat your horse with CONTINUEX® (pyrantel tartrate) Daily Dewormer according
to label directions and administer a program of preventive care
through your veterinarian in compliance with the program guidelines
- CONTINUEX® (pyrantel tartrate) Daily Dewormer can be purchased at any retailer - you aren't required to purchase
through your veterinarian
CONTINUCARE™
Program Eligibility:
Your
horse is not eligible for the CONTINUCARE™ Program if any of the
following apply:
- He/she
is not fully weaned
- He/she
is not at least 5 months old
- He/she
is more than 20 years old
- He/she
has experienced colic at any time during the last 12 months
- He/she
has a history of chronic colic
- He/she
has ever had colic surgery
- He/she
does not have an attending veterinarian
- You
change attending veterinarian after your enrollment becomes
effective:
- Because
your attending veterinarian is so important to your horse's
care, your horse will no longer be eligible for CONTINUCARE™
benefits if you change attending veterinarians after your
enrollment becomes effective.
- You
may re-enroll your horse under a new attending veterinarian,
but you will need to meet all of the eligibility requirements
and complete a new Application for Enrollment and wait for
a new CONTINUCARE™ certificate, just as if you were enrolling
your horse for the first time.
CONTINUCARE™
Program Conditions:
- Each
enrolled horse must live in the United States
- Each
enrolled horse must have an attending veterinarian and must receive
all of the treatments and services listed under HORSE OWNER DUTIES

HORSE
OWNER DUTIES:
- Give
your enrolled horse year-round daily doses of CONTINUEX® (pyrantel tartrate) Daily Dewormer
in accordance with label directions (dated proofs of purchase of CONTINUEX® (pyrantel tartrate) Daily Dewormer are
required)
- Give
your enrolled horse twice yearly treatments of a dewormer
with ivermectin (proofs
of purchase are required)
- Obtain
annual immunizations for your enrolled horse from the attending
veterinarian
- Obtain
annual physical examinations/treatments for your enrolled horse
from the attending veterinarian
- Obtain
annual dental examination/treatments for your enrolled horse
from the attending veterinarian
- Obtain
appropriate nutritional counseling for your enrolled horse from
the attending veterinarian
- Comply
with all of the attending veterinarian's suggested treatments
for your enrolled horse
- Keep
complete records of products and services obtained for your enrolled
horse as required for CONTINUCARE™ benefits (for example, keep
documents proving that your enrolled horse has received his/her
annual physical examination/treatments, annual dental examination/treatments
and appropriate nutritional counseling, and keep dated proofs
of purchase for CONTINUEX® (pyrantel tartrate) Daily Dewormer and dewormer(s) with ivermectin). Upon
Farnam's request, provide Farnam with complete records of the
above products and services obtained for your enrolled horse
- If
your enrolled horse experiences a case of colic that requires
surgery, you must pay all costs associated with the surgery and
aftercare until Farnam Companies, Inc. or its administrator officially
approves the Request for CONTINUCARE™ Reimbursement in accordance
with the terms and conditions of the CONTINUCARE™ Program. Any and all costs
which exceed $5,000 will remain your complete responsibility
for payment
ATTENDING
VETERINARIAN DUTIES:
- The
attending veterinarian must affirm that the horse is under his or her
direct care and that a veterinarian/patient relationship exists
CONTINUCARE™
CERTIFICATES:
- Upon
Farnam's approval of enrollment, Farnam or its authorized representative
will mail a CONTINUCARE™ Certificate to you and to your attending
veterinarian
- Enrollment
does not begin until the date displayed on the CONTINUCARE™ Certificate
- You
will not be eligible for any CONTINUCARE™ Program reimbursement until
the date shown on the CONTINUCARE™ Certificate
CONTINUCARE™
PROGRAM COMPLIANCE:
- You
must fully comply with each of the rules and requirements listed
in the enrollment form
- Failure
to comply shall constitute grounds for Farnam to disqualify your
horse from CONTINUCARE™ Program and you will not receive any reimbursement
under the CONTINUCARE™ Program
CONTINUCARE™
PROGRAM LIMITATIONS:
- CONTINUCARE™
reimbursement will be limited to a maximum of $5,000 per enrolled
horse towards the cost of colic surgery and three days of aftercare
- You
will be solely responsible for any expenses which are not part
of the cost of colic surgery or aftercare, as well as any expenses
over $5,000 and any expenses which you incur for an ineligible
horse
- If
you are a veterinarian or your enrolled horse is fully or partially
owned by a veterinarian, surgeon, or by the immediate family member
of a veterinarian or surgeon, CONTINUCARE™ reimbursement will be
limited to 50% of the cost of colic surgery, with a maximum CONTINUCARE™
reimbursement of $2,500
CONTINUCARE™
REIMBURSEMENT REQUESTS:
- If
your enrolled horse has a case of colic that is serious enough
to require surgery, you must submit an Affidavit of Compliance
and a copy of your horse's medical history to Farnam Companies,
Inc., or its appointed administrator, within five days of the surgery
date
- Also,
a Request for CONTINUCARE™ Reimbursement must be signed and completed
by the attending veterinarian and submitted to Farnam or its appointed
administrator within five days of the surgery date
- Failure
to comply with the five-day time period will result in denial of
CONTINUCARE™ reimbursement
- Farnam
or its administrator reserves the right to review all applications,
purchase receipts, health records and Affidavits of Compliance
prior to making a decision regarding CONTINUCARE™ reimbursement
- Final
decisions on the acceptability of all requests for CONTINUCARE™
reimbursement shall be made by Farnam and its administrator at
their sole discretion
- All
requests for CONTINUCARE™ Reimbursement will be examined for compliance
with the CONTINUCARE™ Reimbursement Program
DISCLAIMER:
- Farnam
assumes no responsibility or liability for the rendering of services
or products not manufactured by Farnam or under Farnam's control
of supervision
PROGRAM
TERMINATION:
- Farnam
may terminate the CONTINUCARE™ Program without any advance notice, and Farnam,
in its sole discretion, reserves the right to disqualify any horse,
horse owner, veterinarian or surgeon from the CONTINUCARE™ Program
