I hereby request that the enclosed photo, which I certify is my original work and which I own all rights to, may be used for the Farnam Companies, Inc. ("Farnam") Website Photo Gallery. I understand that for the use and/or publication of my photo I will receive a $5 gift certificate for Farnam products to be used at my local dealer. I also certify that I am at least 18 years of age and a legal resident of the United States. All subjects in the photo have relinquished any and all rights they may have with regard to use and/or publication of the photo and their likeness or name. All photos become the property of Farnam which reserves the right to publish, edit or otherwise use photos, the likenesses in them, and /or the owner's name for advertising and/or promotional purposes without additional payment to the entrant or any persons or subjects appearing in them. No photographs will be returned. In consideration of Farnam's acceptance of my photo, I hereby agree to indemnify, defend and hold Farnam, its divisions, parent company, subsidiaries, affiliates, agents, employees, successors, and assigns forever harmless from and against any and all liability, claims, and actions for injury or damages or losses of any kind that might be sustained by the acceptance, use, etc. of the certificate.
